To process your application you will need to email evidence of your dogs health to lifelinek9s@gmail.com 

By signing you are accepting full responsibility for your dog, this includes if they cause harm to another dog, human or to themselves during your training with Lifeline K9s.
You have read and accept the terms, conditions and policies. 
You are agreeing to remove your dog from their therapy dog role if they display reactivity, fearful or concerning behaviour, or are attacked by another dog until you have had a consultation.
